Obtaining an FEI is the first step in registering a cosmetic facility with FDA. You need an FEI before you can start the registration process.

How Do I Obtain an FEI?

Your first step is to use FDA's online portal to check whether your firm or facility already has an FEI. The portal can be found at:
https://www.accessdata.fda.gov/scripts/feiportal/index.cfm?action=portal.login

If you can't find your FEI using the portal, you can request one by contacting This email address is being protected from spambots. You need JavaScript enabled to view it.. When requesting an FEI, you will need to provide the following information in your email request:

1. The legal name of the firm being registered.
2. Are you representing the firm as an agent (third party)?
3. Any alternate firm names, including those used for "doing business as" purposes.
4. The physical address of the firm being registered.
5. The designated mailing address for the firm being registered.
6. The name and contact information of the designated contact person at the facility being registered.
7. A comprehensive list of activities conducted at this specific location (e.g., any drug manufacturing, food packaging, etc.).
8. Any other active FDA registration numbers if the establishment also handles foods, drugs, medical devices, or other FDA-regulated commodities requiring FDA registration.
9. Any former names the firm was known by.
10. Any previous addresses linked to the firm.
